Which syllable should the name begin with

In the shastra the name comes from the birth nakshatra. Each nakshatra has four padas and each pada its own syllable — 108 in all. The name is begun with the syllable of the pada the child was born in.

When the Namkaran is done

Traditionally on the eleventh or twelfth day after birth. Some families do it on the sixteenth day or within the first month — whatever the family's custom is, that is what holds.

An auspicious muhurta is chosen for the day, the name is spoken into the child's ear, and called out for the first time before the family.

The custom for Gandmool nakshatras

Ashwini, Magha, Mula, Ashlesha, Jyeshtha and Revati — these six are called Gandmool. Tradition holds the shanti puja on the 27th day from birth, when the Moon comes back to the same nakshatra.

This is not something to fear. It is a custom — and nothing bad attaches to the child's kundali because of it.

The house name and the shastric name

The two can differ, and by tradition they usually do. Keep the shastric name to this syllable; whatever you call the child at home is entirely your own choice — there is no rule on it.
